29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 574 Hartford Turnpike, Shrewsbury, MA: On February 5, 2020 and at times prior, employees were exposed to nip and shear point hazards when they unjammed, set up, and cleaned chocolate manufacturing machines without isolating and locking them out. On January 7, 2020, an employee received a fingertip amputation injury while reaching into a clogged chocolate valve that was not isolated or locked out. On February 5, 2020, multiple employees were exposed while cleaning a Carle chocolate bar molding machine after only isolating the air and electrical energy sources, but not locking them out.
