29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  97 Rindge Road, Fitchburg, MA: On or about February 4, 2020 employees were observed utilizing a vehicle-mounted elevating work platform (aerial Lift) and they were not standing firmly on the floor of the basket.  Employees were climbing onto and over rails to access an upper level exposing them to fall hazards over 25 feet.
