29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) Jobsite: 2205 Gaul St, Philadelphia, PA, first floor, floor hole - Employees conducting framing operations in proximity to the floor hole on the first level, measuring approximately 104" in length, 77" in width, and with a fall distance of 120" (10 feet), were exposed to fall hazards without the use of personal fall arrest systems, covers, or guardrails system.  Observed on or about 2/5/20.
