29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Grinding Department, on or about 2/05/2020:  FAT TUR560 lathe with an attached grinder was not equipped with guarding that prevented the operator from contacting the ingoing nip point of the grinder and roll.  An employee sustained an amputation injury when their hand contacted the grinding wheel and was pulled into the ingoing nip point of the grinder and roll.
