29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    a) Tower International, located at 630 Southwest Street in Bellevue, Ohio; On or about February 13, 2020 the employer did not conduct annual training on the general use of portable fire extinguishers and the hazards involved with incipient stage fires. Employees working in and around welding cells were exposed to hazards associated with small fires.

Employees were exposed to hazards associated with working in areas with elevated sound levels.
