29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:      a) Job site, 3524 Brighton Road, Pittsburgh, PA: On or about February 12, 2020, during roof deck construction related work activities on a roof/patio area, the employer did not ensure that an employee used appropriate fall protection systems/device where a fall hazard of approximately 12 to 14 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 3524 Brighton Road, Pittsburgh, PA: On or about February 12, 2020, during roof deck construction related work activities on a roof/patio area, the employer did not ensure that the ladder was appropriately set up in that it was extended only about 2 feet above the roofline.
