29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)    Building 2750 Heavy Equipment Section: On or about 02/12/2020, an employee's portable workstation contained various electrical equipment that was used with extensions cords connected together and plugged into a wall mounted receptacle outlet.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level:  a)    Building 2750 Heavy Equipment Section: On or about 02/12/2020, employees required to conduct housekeeping work; the employer have not made an initial determination to determine if any employee that disturb lead containing paint chips may be exposed to lead at or above the action level.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes:  a)    Building 1411: On or about 02/12/2020, employees responsible for housekeeping operations in areas where thermal system insulated asbestos containing material exist were not provided with asbestos awareness training.   Note: Employees who performed housekeeping operations in areas containing presumed asbestos-containing material (PACM) and/or other asbestos-containing material (ACM) are required awareness training at least once a year.   No abatement certification or documentation required.
