Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the lifting devices fail:    a)  Anodize Area, on or about 2/11/2020: A 1 ton CM Loadstar Hoist No. 9 had not been inspected on a frequent and periodic basis.    b) Zinc-Nickel Area, on or about 2/11/2020: A 1/2 ton CM Loadstar Hoist No. 3 had not been inspected on a frequent and periodic basis.    c) Eastside Oxide Area, on or about 2/11/2020: A 1/2 ton CM Loadstar Hoist No. 7 had not been inspected on a frequent and periodic basis.    d) CAD Plating Area, on or about 2/11/2020: A 250 lb CM Shopstar Hoist No. 75 had not been inspected on a frequent and periodic basis.

29 CFR 1910.147(c)(6)(ii): The employer had not certified that periodic inspections of the energy control procedures had been performed:   a) At the establishment, on or about 2/11/2020: Certification that periodic inspections had been accomplished on the energy control procedures for employees performing servicing and/or maintenance on various equipment, including but not  limited to:  Quincey Small Baking Oven; Grieve Big Oven; Big Steam Boiler; Hot Water Boiler; Small Steam Process Boiler Air Compressors #1 and #3; Air Dryer #1; Roof Top Generator and Chiller had not been completed.
