29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about February 13, 2020 located in the Hone and Lathe shop, where an employee was exposed to struck by and caught in hazards when using a Kuraki KH6-30 / T62 lathe without a chuck guard.  b) On or about February 13, 2020 located in the Bore and Drill shop, where an employee was exposed to struck by and caught in hazards when using a Smalley/DB18-4 lathe without a chuck guard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   a) On or about February 13, 2020 at the Bore and Drill shop an extension cord used to power an Airmaster pedestal fan was pulled from the strain relief, exposing employees to electrical hazards.
