29 CFR 1926.760(d)(2): Fall arrrest system components were not used in fall restraint systems and did not conform to the criteria in 1926.502 (see Appendix G):  a) On or about February 13, 2020 and times prior, for a jobsite at 2100 Moores Lane Mulberry, FL.  An employee engaged in the replacement of metal roof decking and exterior wall was using a personal fall arrest system with the lifeline not protected from being cut, exposing to fall hazard of approximately 27 feet to the ground below.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section:  a)  On or about February 13, 2020, and times prior to, for a jobsite located at 2100 Moores Lane in Mulberry, Florida.  Employees with improper personal fall protection equipment were exposed to a 30 foot fall while engaged in metal decking roof repair and metal wall panel repair.
