This violation was observed on or about February 12, 2020, the employer did not ensure that the work rests were adjusted to ensure a maximum opening of one eight inch. This exposed employees to caught in hazards: 1) At the Dayton bench grinder, Model 4Z672E, there was a gap of ½ inch between the right grinding wheel work rest and the grinding wheel. 2) At the Baldor bench grinder, Model 4Z672E, there was a gap of ¼ inch between the right grinding wheel work rest and the grinding wheel.

This violation was observed on or about February 12, 2020, the employer did not ensure that the distance between the abrasive wheel periphery and the adjustable tongue did not exceed one fourth inch: 1) At the Dayton bench grinder, between the tongue guard and the grinding wheel was a gap exceeding one fourth inch exposing employees to the hazards of being struck by a shattered grinding wheel. 2) At the Baldor bench grinder, between the missing tongue guard and the grinding wheel was a gap exceeding one fourth inch exposing employees to the hazards of being struck by a shattered grinding wheel.

This violation was observed on or about February 12, 2020, the employer did not ensure that an extension cord located in the King Vertical Boring Mill was not used as a substitute for fixed wiring. An electrical cord for a makeshift light mounted an a metal pole was plugged into the extension cord and this exposed employees to electrical hazards.
