29 CFR 1910.23(b)(10): The employer did not ensure any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language and removed from service until repaired in accordance with 1910.22(d), or replaced.   On or about February 13, 2020, and at times prior thereto, in the scene shop, the employer did not ensure an A-frame ladder with a damaged rung was immediately tagged and removed from service until repaired or replaced.  This exposed the employees to a fall hazard.

29 CFR 1910.23(e)(1)(vii): Mobile ladder stands and platforms that have wheels or casters were not equipped with a system to impede horizontal movement when an employee is on the stand or platform.  On or about February 13, 2020, and at times prior thereto, in the scene shop, the employer did not ensure that the casters on a mobile ladder were equipped with an operable locking system to prevent horizontal movement when in use by employees.  This exposed the employees to a fall hazard.

29 CFR 1910.28(b)(1)(i):  Each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was not protected from falling by standard railings; safety net systems; or personal fall protection systems.   On or about February 13, 2020, and at times prior thereto, in the Kirkpatrick Theater, the employer did not ensure that employees, performing lighting installation work activities to the front of a balcony, were provided fall protection.  This exposed the employees to a fall greater than four(4) feet to the ground below.

On or about February 13, 2020, and at times prior thereto, the employer did not ensure that an employee, required to use a personal fall protection system, was trained to properly use the fall protection prior to use.  This exposed the employees to a fall hazard.
