29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) On or about February 12, 2020, and at times prior, the employer did not ensure that work rests on grinding machinery were adjusted closely to the wheel with a maximum opening of one eighth inch. Employees of Dionisio Metals Recycling were exposed to struck-by hazards, in that the opening between the abrasive wheel of a  5" Central Machinery model 907 grinder and the work rest was 1 inch. Employees of Dionisio Metals Recycling were exposed to contact with electrical hazards, in that receptacles with unused openings were not effectively closed exposing the live wiring as follows:      1- Receptacle on the North wall of the processing area had two unused openings.      2- Receptacle on the West wall of the processing area had one unused opening.      3- Receptacle on the South wall of the storage area had one unused opening. Employees of Dionisio Metals Recycling were exposed to contact with electrical hazards in that the employer used flexible cords as a substitute for permanent wiring as follows:      1- A flexible cord was used to provide power to 120V scale and equipment in the customer intake area.      2- A flexible cord was used to provide power to 120V equipment for the aluminum can conveyor/hopper in the outdoor yard.
