29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about 2/13/2020, at 2800 SW 2nd Ave. Fort Lauderdale, FL 33315, employees were working on a roof at 14 feet above ground with unprotected skylights, exposing them to a fall hazard.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about 2/13/2020, at 2800 SW 2nd Ave. Fort Lauderdale, FL 33315, the employer did not develop, implement, and maintain a written hazard communication program while using hazardous chemicals for roofing work such as, but not limited to, roofing asphalt Bitumar Type 4.
