29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  USS Lake Champlain, Sonar 1: Employees performing fire watch tasks were required to use half-mask respirators during hot work and needle gun operations. The exposure determination / air monitoring conducted at the site was for another employer's workers performing surface preparation and paint removal operations. Employees were exposed to respiratory irritation hazards associated with metal fumes and/or particulates.

An employee wore the tight-fitting respirator with identifiable facial hair that protruded from the facepiece, which exposed him to respiratory hazards associated with metal fumes and/or particulates.
