Employees engaged in residential roofing activities, walking and working on a approximately 4:12 pitch roof, were exposed to fall hazards of approximately 12 feet from the eave of the residence to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employees engaged in residential roofing activities were exposed to falls from approximately 12 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof.
