The employer failed to instruct each employee in the recognition and avoidance of unsafe condition(s) such as the use of appropriate fall protection, the safe use of ladders, and the use of personal protective equipment specifically eye protection, on or about February 12, 2020.  Abatement certification required within 10 days after abatement date. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) 300 Sky Port Road Mechanicsburg, PA  - Employees installed and removed forms, and vibrated concrete for a 8 inches wide residential foundation, without the benefit of a fall protection system, exposing employees to a 10 foot fall from the top of the form to the ground below, on or about February 12, 2020.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
