Employees engaged in material handing activities using the K-D Manitou forklift, truck model M230CP, serial number M2300452, were exposed to struck-by hazards in that the employer did not provide comprehensive forklift training prior to permitting employees to operate forklifts.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced:  Employees were exposed to electrical shock hazard.  Employees engaged in welding activities in the shop area were exposed to electrical shock hazards in that the Miller arc welder, model Millermatic 252 cables had damaged insulation, exposing bare conductors.  The welder was located in the shop area, Eastside wall, and by the walkway door.
