29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's requirements: New townhome construction located on Lot #10, Front side: On or about 02/19/2020, and at times prior, the employer exposed employees to collapse/fall hazards of 23-feet to the lower level, in that the tubular welded scaffold used to install bricks was not tied into the structure to prevent tip over and/or collapse of the system.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: New townhome construction located on Lot #10, Front side: On or about 02/19/2020, and at times prior, the employer exposed employees to fall hazards of 23-feet to the lower level, in that the tubular welded scaffold used to install bricks was not provided with mid, top , and side rails along all open sides of the scaffold platform.
