On the rooftop of Building 5, located at 1462 Solera Terrace, Jacksonville FL: On or about February 20, 2020, the employer exposed employees to a 19-foot 9 inches fall hazards, in that fall protection was not utilized by employees performing framing work.     b. On the rooftop of Building 3, located at 1462 Solera Terrace, Jacksonville FL: On or about February 20, 2020, the employer exposed employees to a 19-foot 9 inches fall hazards, in that fall protection was not utilized by employees performing framing work.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a. On the east side of building 5 at 1462 Solera Terrace, Jacksonville FL: On or about February 20, 2020 and at times prior, the employer exposed employees to struck-by hazard because the forklift operator was not trained, evaluated and certified for the operation of the equipment.
