29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  a)    New MRI facility, Hampton VA: On or about 02/20/2020, an employee used a pedestal light with an extension cord plugged into a 120-volt wall receptacle without a ground fault circuit interrupter device as a result was potentially exposed to a shock hazard.   No abatement certification or documentation required.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a)    New MRI facility, Hampton VA: On or about 02/20/2020, the employer did not prepare and provide a written certification record for employees required at work on a low sloped roof approximately 12ft above the ground.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
