29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: a) At the jobsite the employer did not ensure that extension cords and power tools used on the construction site were inspected for damage prior to being used, exposing employees to the hazard of electrical shock.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws: a) At the jobsite, the 25-foot red/black extension cord used to supply power to the Ridgid 300 Pipe Threader had the sheathing pulled away from the female end of the cord. b) At the jobsite, the power cord for the Ridgid 300 Pipe Threader had the outer sheathing pulled from the plug end.
