29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:    Employees were exposed to fall hazards.  Employees working form the scissor lift and walking/working during installation activities were exposed to fall hazards.  Employees engaged in fire suppression installation in the convention center area were exposed to fall hazards in that holes, approximately 6 feet in length, 3 feet 1.5 inches in width, and 4.5 inches in depth, on the concrete floor were not covered.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    Employees were exposed to slip and fall hazards.  Employees engaged in construction activities using the eastside stairway were exposed to slip and fall hazards of various heights up to approximately 11 feet from the walking/working surface of the stairway to the ground below.  Employees were exposed in that no handrail was provided.
