29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Stock Room and Store: On or about February 28th, 2020, employees were exposed to struck by hazards while working where unsecured compressed gas cylinders were transported from the stock room to the front of the store on box carts where a device to prevent the cylinder from tipping or falling off was not utilized.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: Store Restroom Area and Stock Room: On or about February 28, 2020 employees were exposed to hazards associated with a delayed emergency evacuation of the building while working where boxes of merchandise, store equipment, and empty cardboard boxes obstructed the exit routes to less than the minimum width requirement of 28 inches in the following locations: a) In front of the employee restroom - 21 inches b) In front of the customer restroom - 21 inches c) In front of the Break Room Area - 24 inches d) In front of the Utility corridor - 24 inches The Dollar Tree Stores, Incorporated was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 967705, citation number 1, item number 1 and was affirmed as a final order on January 28, 2016, with respect to a workplace located at 8028 Denton Highway, Watauga, TX 76148.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: Stock Room: On or about February 28, 2020, employees were exposed to struck by hazards while working in the stock room where boxes of merchandise were not stacked securely to prevent sliding and/or from falling on employees in the following locations: Instances: a) The rear wall of the stock room. b) The stock room entry corridor. c) The store side wall of the stock room in front of the emergency exit/ receiving door. d) The Helium storage wall and the adjacent area in the stock room. Dollar Tree Stores, Inc. was previously cited for a repeat violation of this occupational safety and health standard which was contained in OSHA inspection number 1269761, citation number 1, item number 2a issued on June 14, 2018, and was affirmed as a final order on September 7, 2018, with respect to a workplace located at 1155 S. Havana Street, Unit 23, Aurora, CO 80012.
