29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    On or about February 17, 2020, at a building renovation site located at 540 S. Victor in Tulsa, OK, employees were allowed to work on a scaffold that did not have all working levels fully planked, exposing the employees to the hazard of falls.

Victor in Tulsa, OK,  Employees were observed working on a scaffold that did not have guardrails around all open ends, exposing the employees to the hazard of falls.
