29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    On or about February 18, 2020, at an apartment complex re-roofing site located on the corner of 21st and Boston in Tulsa, OK, the employer did not ensure that employees, performing roofing work activities from a roof, were in use of fall protection.  This exposed the employees to a fall hazard greater than six(6) feet to the ground below.

29 CFR 1926.1053(b)(1):  A portable ladder used for access to an upper landing surface did not have the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension is not possible because of the ladder's length was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.      On or about February 18, 2020, at an apartment complex re-roofing site located on the corner of 21st and Boston in Tulsa, OK, the employer did not ensure that the side rails of an extension ladder extended at least three (3) feet above the landing.  Employees used this ladder to access the roof and were exposed to a fall hazard.
