29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Re-man area, Moulder machines 1-3; On or about 25 February 2020 and at times prior to, the employer exposed employees to potential unexpected energization of equipment in that employees were allowed to conduct set-up and tool change operations on moulder machines #1-3 without locking out the equipment. The employer allowed employees to rely on control circuitry (E-stop/key selector) as the energy isolating device which is not allowed by the standard. The employer failed to ensure employees utilized the machine specific lockout tagout procedures for those machines.
