1910.147(c)(5)(i)  Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources.    a) Wilson Excavating, 407 East Nashannock Avenue, New Wilmington, PA, equipment storage yard.  On or about February 28, 2020, employee using the Down 2 Earth Utility Trailer was exposed to struck-by/ caught-between hazards and crush hazards created by placing an employees body underneath the trailer while performing activities that included but were not limited to coupling and/or uncoupling the trailer.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

The sign did not contain sufficient information to be easily understood:  a) Wilson Excavating, 407 East Nashannock Avenue, New Wilmington, PA, equipment storage yard.  On or about February 28, 2020, the warning decals and signage on the Down 2 Earth Utility Trailer were not legible.  Warning decals were worn and peeling off the trailer making the signage difficult to read.
