29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about February 28, 2020 at Tubman Elementary School in the courtyard employees utilized extension cords that were damaged. Employees were not trained to recognize and avoid unsafe conditions or hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about February 28, 2020 at Tubman Elementary School in the gymnasium employees utilized an extension cord for a Hitachi circular saw that did not have adequate strain relief.
