29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. On or about February 27th, 2020, and at times prior thereto, employees were working on pump jack scaffolding in order to install fascia boards to the exterior of a residential construction site. The poles of the pump jack scaffolding were not attached to the structure at the bottom of the poles, and were only attached to at the top by plywood, exposes employees to the hazard of falling from height.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about February 27th, 2020, and at times prior thereto, employees were working at heights of greater than 6 feet while not utilizing any form of fall protection, exposing employees to the hazard of falling from height.
