29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface:    (a)    Production Area:  On or about July 21, 2020 employees were not protected from a fall hazard by a standard height railing of a minimum of 39 inches high while standing on portable platforms located on the top platform of the hopper that feeds powdered naphthalene to the ball presses.  The guardrail height was reduced to 32 inches high while standing on the portable platforms.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)  Production Room next to Ball Presses:  On or about July 21, 2020 an Airmaster floor fan was plugged in on a continuous basis using an extension cord as a substitute for the fixed wiring of the building.  (b)  Production Room next to Debagger Operation:  On or about July 21, 2020 an Airmaster floor fan was plugged in on a continuous basis using an extension cord as a substitute for the fixed wiring of the building.
