29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on March 3, 2020 on the jobsite located at 9763 Westchester Dr., Omaha, NE 68114.  There were 6 employees re-roofing an existing 2-story residential house and not utilizing any form of fall protection.  The house has a 6 in 12 steep sloped pitch and ground to eve height is approximately 9 feet on the garage section and 18.5 feet on the house portion.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:   The employer is failing to ensure that all employees are protected from fall hazards when using an A-frame ladder in the closed position.  This most recently occurred on March 3, 2020 on the jobsite located at 9763 Westchester Dr., Omaha, NE 68114.  Employees are using an A-frame ladder to get from the roof of the attached garage to the roof of the 2-story house.  The ladder is being used in the closed position; therefore, the locking device is not being used.

This most recently occurred on March 3, 2020 on the jobsite located at 9763 Westchester Dr., Omaha, NE 68114.  An employee is using an A-frame stepladder to access and egress the roof of a 2-story residence.  The ladder is placed on an uneven surface and not secured to prevent accidental displacement.

This most recently occurred on March 3, 2020 on the jobsite located at 9763 Westchester Dr., Omaha, NE 68114.  Employees are using an aluminum extension ladder to access and egress the garage portion of the roof where both of the side rails are broken at the top of the ladder.
