29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jesus Carrillo Trimmer at 9852 Reunion Pkwy. Commerce City, CO 80022: On or before 3/4/2020 the employer did not protect employees from injury in that an employee installing pump jack scaffold brackets on the roof of a two story single family home was not utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 12 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jesus Carrillo Trimmer at 9852 Reunion Pkwy. Commerce City, CO 80022: On or before 3/4/2020 the employer did not train employees on fall protection in accordance with this standard. An employee installing pump jack scaffold brackets on the roof of a two story single family home was not utilizing a fall protection. This condition exposed employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jesus Carrillo Trimmer at 9852 Reunion Pkwy. Commerce City, CO 80022: On or before 3/4/2020 the employer did not protect employees from injury in that an employee used an extension ladder which only extended approximately 1.5 feet past the eave of the roof. The ladder was not placed securely and the employee nearly fell off the ladder. This condition exposed an employee to fall hazards.
