29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards.    (a)  On or about March 4, 2020, the employer did not ensure the operator of the Dur-A-Lift Bucket Truck (Model No. DTA 35 & Serial No. A4972) was trained to operate the bucket truck.  This exposed employees to struck by and fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about March 4, 2020, employees conducting sheathing activities on the roof of a newly built detached garage with a 4:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet.
