29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily: a) On or about March 3, 2020, and at times prior, the employer did not ensure that combustible waste material and residues in a building were kept to a minimum, stored in covered metal receptacles and disposed of daily. Employees were exposed to fire hazards in that oily rags were kept overnight in open 30-gallon plastic containers. Containers with oily rags were not disposed of daily. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) On or about March 3, 2020, and at times prior, the employer did not ensure that work rests on grinding machinery were adjusted closely to the wheel with a maximum opening of one eighth inch. Employees were exposed to struck-by hazards, in that a DeWalt 8" grinder was used while the work rest was not adjusted properly. The distance between the abrasive wheel and the work rest was 1".
