29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a)    Adjacent to 101 Centerton Rd, Mount Laurel, NJ, Southeastern area of site - An employee did not have a proper means of access and egress while working on his hands and knees, at the bottom of the unprotected excavation with nearly vertical walls, approximately 6'5" deep x approximately 5' wide x approximately 8-10' long; employee had to use his hands and feet to climb the loose sandy soil to get out of the excavation, on or about 02/25/2020.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):    a)    Adjacent to 101 Centerton Rd, Mount Laurel, NJ, Southeastern area of site - An employee was working on his hands and knees, at the bottom of an unprotected excavation with nearly vertical walls, approximately 6'5" deep x approximately 5' wide x approximately 8-10' long, on or about 02/25/2020.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
