29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):      a) Production - On March 4, 2019, the employer did not ensure that the guard was properly adjusted to cover all unused portions of the DoAll vertical band saw blade, such as the unused portion of the point of operation.    b) CNC Department - On March 4, 2019, the employer did not ensure that the guard on the Fadal VMC 3016 Vertical Milling CNC was properly designed to prevent employees from having any part of their body in the danger zone(s) during the operating cycle.    No abatement certification or documentation is required for this item.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a) Warehouse- On March 4, 2020, the employer did not ensure that an evaluation of the operator's knowledge and skill to operate the power industrial vehicles safely was performed every three years for each type of vehicle operated, such as the Clark sit down forklift, Model #TMC15, Serial #TMC 248-1134-9570KF.   No abatement certification or documentation is required for this item.
