29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccination series to all employees who have occupational exposure:   Establishment: Employer is exposing employees to hazardous conditions.  The employees are permitted to work where they are potentially exposed to bloodborne pathogens without being provided the required Hepatitis B vaccination series.   On 03/05/2020 employees who were potentially exposed to bloodborne pathogens were not offered the required Hepatitis B vaccination series.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   Establishment: Employer is exposing employees to hazardous chemicals.  The employees are permitted to use hazardous chemicals in the workplace without receiving required hazard communication training.   On 03/05/2020 employees used hazardous chemicals such as, but not limited to, Acetone without having received required hazard communication training.

29 CFR 1910.1020(g)(1): Employees covered by 29 CFR 1910.1020 were not informed, upon entering employment and at least annually thereafter, of the existence, location, availability of any records covered by this section, of the identity of the person responsible for maintaining and providing access to records, or of each employee's right of access to these records:   Establishment: Employer is not informing employees of their rights of access to their medical and exposure records.  Employees who had records such as, but not limited to, Hepatitis B vaccination records and safety data sheets for chemicals in use, were not provided the required information.
