29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   a)  Paint Department, on or about 3/2/2020:   3M Full-face and Survivair half-mask tight fitting respirators are required to be worn by an employee when spray painting.   The employer did not provide a medical evaluation to determine the employee's ability to use a respirator before the employee was fit tested and used the respirators in the workplace.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:   a)  Production Area, on or about 3/5/2020:   Distance between the tongue guard and the abrasive wheel periphery on Jet Grinder, Model JBG-8A, measured approximately 1-1/8 inches.  b)  Welding Area, on or about 3/5/2020:  Distance between the tongue guard and the abrasive wheel periphery on Delta Grinder, measured approximately 7/8 inches.
