29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Location: Raynham Preserve Residential Development On or about March 5th, 2020 employees were exposed to struck-by over-head hazards while guiding the rigged material without the use of a tag line and walking directly under the load without protection from head injuries

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). Location: Raynham Preserve On or about March 5th, 2020 employees were exposed to falls from heights of approximately eight to twelve feet to the ground while working on a residential new construction development where fall protection was not utilized in the following instances: 1) Framers preparing to fly in the walls in the 2nd floor deck 2) Roofers on the roof waiting for shingles to tie off
