29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) Future Site of O'Reilly Auto Parts (South Side: Front Area): On or about March 04, 2020, the employer allowed exposed employee(s) to perform framing activities on a pallet platform that was elevated by a JLG 6042 Skytrack Telehandler forklift; exposing employee(s) to fall hazards of approximately twenty three (23) feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Future Site of O'Reilly Auto Parts (South Side: Front Area): On or about March 04, 2020, the employer failed to ensure exposed employee(s) were using a body belt with lanyard attached to the approved anchor point while using a Genie S-65 telescopic boom aerial lift to perform framing activities; exposing employee(s) to catapult hazards and fall hazards of approximately twenty three (23) feet to the ground below.
