29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Instance a)  34 Keyes Street, Florham Park, NJ.  Employees involved with rough framing of a new residential home were exposed to head injury from framing materials when work was occurring overhead as no worker wore a hard hat.                                        Violation observed on or about 03/07/2020.

29 CFR 1926.1053(b)(1):   Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Instance a)  34 Keyes Street, Florham Park, NJ.  Employees involved with rough framing of a new residential home were exposed to falls of up to 10 feet as they were accessing the second floor ceiling joists at least 10' above the second floor with a 6' stepladder.                                       Violation observed on or about 03/07/2020.

Employees involved with rough framing of a new residential home were exposed to falls of up to 22' from the third floor's joists to the ground and to falls of 10' to the second floor as fall protection was not being used.                                      Violation observed on or about 03/07/2020.
