29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) Hartman Building located at 1627 St RT 271 North, Hawesville, KY: On or about March 5, 2020, an electrical control box cover was not secured exposing employees to electrical hazard.

29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees are exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which present a hazard of falling more than 4 feet (1.22 m) or into the water, except as specified in paragraph (b)(2) of this section.   a)  Hartman Building located at 1627 ST RT 271 North, Hawesville, KY  waters edge on LEFT and RIGHT side of initial entry onto gangway of Level 1:  On or about March 5, 2020, a portion of the loading / unloading  platform was unprotected exposing workers to a fall hazard.
