29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guiderail system and the walking-working surface:  Exterior Loading Dock:  On, or about, March 6, 2020, the guardrail system that had been installed along the leading edge of the loading dock did not have a properly positioned mid rail.  A gap of 27 inches was left between the walking surface and midrail member.

29 CFR 1910.110(b)(17)(v):  Open flames, cutting or welding, portable electric tools and extension lights capable of igniting LP Gas were permitted within classified areas specified in table H-28 of this section:  Warehouse, Compressed Gas Cylinder Cage:  On, or about, March 6, 2020, a No Smoking sign or similar warning had not been posted where flammable compressed gases such as propane and acetylene were stored.
