Employees were exposed to electrical shock hazards.  1. An Electrical Extension Cord connected to an Electrical Outlet Box and Outlet  2. An Electrical Power Strip was used to charge power tools and an extension cord to power metal shears.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws :  On or about February 28, 2020, and at times prior thereto, at a commercial construction jobsite, the employer did not ensure electrical equipment was provided strain relief. Employees were exposed to electrical shock hazards.  1. An Electrical Extension Cord connected to an Electrical Outlet and Outlet was not provided with strain relief. 2. An Electrical Power Strip used to charge power tool batteries and an extension cord to power metal shears was not provided with strain relief.

On or about February 28, 2020, and at times prior thereto, at a commercial construction jobsite, the employer did not ensure employees elevated by shop made scaffold platform by a Genie GTH 842 Rough Terrain Forklift were provided fall protection. The employees were exposed to unprotected fall hazards greater than (10) feet to the ground below.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: (General Industry Reference 1910.27(a))   On or about February 28, 2020, and at times prior thereto, at a commercial construction jobsite, the employer allowed employees to work from an elevated shop made scaffold platform that was NOT approved for use on the Genie GTH 842 Rough Terrain Forklift. The employees were exposed to fall hazards greater than (10) feet to the ground below.
