29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  On or about March 5, 2020, at the job site, the employer did not ensure that an electrical extension cord was protected from damage including but not limited to the following:    1. The cord was laid on the ground across a driveway with vehicle traffic. 2. The cord was run through doorways.     This exposed employees to electric shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about March 5, 2020, at the job site the employer did not ensure that employees, working from the roof of a building installing framing materials, were protected from falling.  The employees were not in use of fall protection and were exposed to a fall hazard greater than six (6) feet to the ground below.
