29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames did not have a maximum spacing between rungs of 16 3/4 inches (43 cm):  a)    Northeast end of building, 101 Centerton Rd, Mount Laurel, NJ:  Workers climbed the end frame to access the platform, approximately 15-16 feet above ground level, rungs measured approximately 19 5/8" from top of one to top of the next, on or about 02/25/2020.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)    Northeast end of building, 101 Centerton Rd, Mount Laurel, NJ:  An employee without any fall protection approximately 15-16 feet above ground level, was dismantling the approximate 16 foot wide by 20 foot high by 5-9 foot deep fabricated frame scaffold, on or about 02/25/2020.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
