In the rooftop at the front of the house, located at 2021 Traymore Road, Jacksonville FL: On or about March 10, 2020, the employer exposed an employee to struck-by hazard of flying debris or nails, in that eye protection was not utilized by employee while operating a pneumatic nail gun.    b. In the rooftop at the back of the house, located at 2021 Traymore Road, Jacksonville FL: On or about March 10, 2020, the employer exposed an employee to struck-by hazard of flying debris or nails, in that eye protection was not utilized by employee while operating a pneumatic nail gun.

In the rooftop at the front of the house, located at 2021 Traymore Road, Jacksonville FL: On or about March 10, 2020, the employer exposed two employees to a 9 foot 7 inches fall hazards, in that fall protection was not utilized by employees performing roofing work.     b. In the rooftop at the back of the house, located at 2021 Traymore Road, Jacksonville FL: On or about March 10, 2020, the employer exposed an employee to a 9 foot 7 inches fall hazards, in that fall protection was not utilized by employees performing roofing work.
