1910.9(b) Training. Standards in this part requiring training on hazards and related matters, such as standards requiring that employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, impose a separate compliance duty with respect to each employee covered by the requirement. The employer must train each affected employee in the manner required by the standard, and each failure to train an employee may be considered a separate violation. [FR 73 75583, Dec. 12, 2008] a. On or about March 10, 2020, at the above addressed site, a host employee and contract employees were removing a hub on the drive-end of a locomotive generator/alternator (Kato Engineering Model number AA2907300) using an Enerpac, model number RR-1006, double action hydraulic cylinder which was connected to a Enerpac, model number PER-2222, 10,000 pound hydraulic pump. The coupler on the return side of the hydraulic cylinder was not provided with a hose, nor was there a relief valve or pressure gauge installed on the system. Employees and contract employees were thereby exposed to struck-by hazards due to catastrophic failure of the components. An employee was struck by and fatally injured by a failure of the system.
