On the conveyor motor where employees were exposed to caught-by hazards while working near a shaft end that was part of the drive assembly for the conveyor line leading to the chipper without a guard to cover the shaft end or the shaft projecting less than one half the diameter.       b. On the belt and pulley assembly on the trimmer line where employees were exposed to struck-by and caught-by hazards while working near a shaft end that was part of the drive assembly for the trimmer without a guard to cover the shaft end or the shaft projecting less than one half the diameter.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.    On or about February 20, 2020 an employee working adjacent to an exposed belt and pulley on a trim saw suffered injuries and was hospitalized without the employer reporting the incident to OSHA within 24 hours.
